• Gather Patient Information: Collect and review patient medical records, diagnosis codes, and procedure details to support the pre-authorization request.

• Verify Insurance Coverage: Confirm patient information and insurance coverage through verifying photo ID, insurance cards, and personal information.

• Submit Pre-Authorization Requests: Prepare and submit pre-authorization requests to insurance companies, adhering to their specific procedures and requirements.

• Follow Up: Monitor the progress of pre-authorization requests and follow up with insurance providers to expedite the approval process.

• Communicate with Healthcare Providers: Collaborate with healthcare providers to obtain any necessary documentation, notes, or additional information required for pre-authorization.

• Patient Education: Explain the pre-authorization process to patients, including potential out-of-pocket costs, and address any questions or concerns they may have.

• Document and Recordkeeping: Maintain detailed records of pre-authorization requests, approvals, denials, and communications with insurance companies and healthcare providers.

• Compliance: Ensure that all pre-authorization processes comply with relevant laws and regulations, including HIPAA (Health Insurance Portability and Accountability Act).

• Appeals: If a pre-authorization request is denied, assist in the appeals process by providing necessary documentation and information to support the appeal

• Safely and efficiently operates and maintains a variety of equipment, which may include: semi-tractors, semi-trailers, pup-trailers, straight trucks, forklift, low boy, end dumps and various hand tools.

• Loads trucks and secures loads for shipment in a safe and efficient manner in accordance with branch procedure.

• Fuels and cleans trucks.

• Delivers and picks up loads in a timely manner.

• Produces accurate, timely, and legible paperwork.

• Complies with DOT, OSHA and branch safety standards.

• Performs vehicle safety inspections and load checks as required.

• Advertises for the company when on the road.

• Performs maintenance and simple repairs in a timely, effective manner and schedules other repairs and maintenance.

• Assists in the recycling yard as needed.

• Demonstrates commitment to be present and work all hours assigned as needed to complete work orders.

• Willingly performs and completes any assigned work in an efficient, safe & productive manner.

Local concrete company is currently seeking a skilled employee to assist formsetters with various duties. 

• The ideal candidate will be responsible for preparing work sites for new concrete installations, designing and constructing forms for walls, structures, or pillars, mixing and pouring concrete, as well as using tools to spread and smooth the concrete. 

• Apply finishing coats, stains, or polish to the concrete, install rebar, expansion joints, or carbon fiber, and repair concrete structures as needed. 

• Follows all safety protocols when using tools and equipment, and ensures that the work site is cleaned up upon project completion.

• Understanding safety hazards of each job and maintain proper personal protective equipment. 

• Drill holes in concrete with rock drill at properly analyzed locations.

• Mixing of slab jack material by shoveling and lifting material into mixer and dumping properly mixed material  into mud pump and operating mud pump.

• Properly mixing and matching repair material to fill in grout holes.

• Clean up job site and equipment. 6. Perform preventive maintenance on equipment

• This position is responsible for safely operating a triaxle tanker or transport with a 48 foot tank from a water source to a customer location to either pick up or deliver water.

•  Prior experience in construction work and familiarity with hand and power tools is required.

• Concrete Form Setters must be comfortable working outside in all types of weather conditions and lifting heavy objects.
